There's exactly one doctor (LNA) that cannot act as a normal emergency doctor. This guy isn't able to serve as you like him to do. Try to get any other doctor and things will work out for you 
Another problem could be caused because of misunderstanding of the german EMS-System.
On a standard ambulance car, there's a paramedic and an EMT serving. For the sake of gameplay, they changed that to two Paramedics working on an ambulance.
Whilst paramedics being pretty skilled, it's not enough to treat they "real" medical emergencies. So... call a doctor, when the icon pops and a clicking sound appears
